If a research/trial was funded by sponsor or outside the TMU system is applying for amendment, a charge of NTD 3,000 should be submitted. If a research/trial was funded by government or the relationship with TMU system is applying for amendment, a charge of NTD 1,000 should be submitted. (Not include Withdraw)

The complete file should include the forms with signature below:

2	Protocol Abstract
3	Summary Table of Protocol Withdrawn/Halt/Stop
4	Review Form of Protocol Withdrawn/Halt/Stop
5	Approved Empty ICF(NA for Protocol Withdraw)
6	Copied ICF with Participants’ signature(NA for Protocol Withdraw)
7	SAE Record Sheet
8	All previous approval letter
9	Others, e.g. Proof of Payment of Review Fee______________________
