If a research/trial was funded by sponsor or outside the TMU system is applying for amendment, a charge of NTD 5,000 or NTD 10,000 should be submitted (depends on the frequency of report). If a research/trial was funded by government or the relationship with TMU system is applying for amendment, a charge of NTD 500 or NTD 1,000 should be submitted (depends on the frequency of report) should be submitted.

The complete file should include the forms with signature below:

1	List of Application Document
2	Protocol Abstract
3	Continuing Report Form
4	Recommendation Sheet of Continuing Report
5	Approved Empty ICF
6	Copied ICF with Participants’ signature
7	SAE Record Sheet (if applicable)
8	Declaration Form of Significant Financial Interest and Non-Financial Interests
9	All previous approval letter
10	Others, e.g. Proof of Payment___________________________
