Submit Final Report

If you're TMU's international student or faculty and need to submit a study, please contact us by email (tmujirb@gmail.com) to request.

If a research/trial was funded by sponsor or outside the TMU system is applying for amendment, a charge of NTD 10,000 should be submitted. If a research/trial was funded by government or the relationship with TMU system is applying for amendment, a charge of NTD 1,000 should be submitted.

The complete file should include the forms with signature below:

1	List of Application Document
2	Protocol Abstract
3	Final Report Form
4	Recommendation Sheet of Final Report
5	Approved Empty ICF(NA for Protocol Withdraw)
6	Copied ICF with Participants’ signature(NA for Protocol Withdraw)
7	SAE Record Sheet (if applicable)
8	All previous approval letter
9	Others, e.g. Proof of Payment of Review Fee______________________
