If a research/trial was funded by sponsor or outside the TMU system is applying for amendment, a charge of NTD 5,000 should be submitted. If a research/trial was funded by government or the relationship with TMU system is applying for amendment, a charge of NTD 1,000 should be submitted.

The complete file should include the forms with signature below:

2	Protocol Abstract
3	Amendment Instruction Sheet
4	Recommendation Sheet of Amendment
5	Cross-reference list (Comparison table)
6	Documents before Revision
7	Documents after Revision
8	All previous approval letter
9	Others, e.g. Proof of Payment of Review Fee___________________
